All member registration information is treated as confidential. It is not sold, shared or disclosed to unaffiliated parties not directly related to Program administration or customer service.
What Information We Collect:
Member information is limited to that which is necessary to provide services to registered members. For standard group programs, this usually is limited to name, birth date, gender, dependent status, email address, address, telephone number, and employer, association or group.
For non-funded programs, the information is usually limited to name, and a coded member ID number. Health Plan Partners, LLC does not collect, receive or have access to personally identifiable health or pharmaceutical information.
We are contracted with Elixir Savings (Elixir) for certain administrative support services necessary to provide our programs and services to members.
Elixir Savings and its affiliates and subsidiaries may contact you via e-mail to:
- Respond to any questions you may send us;
- Send you e-mail communications about information we think you will find useful – including announcements about new products and services at this Site; or
- Communicate other information as necessary to administer this Site.
To Whom We Disclose:
All member information is maintained in strict confidence and is utilized solely for administrative functions directly related to the provision of our programs and services.
We are contracted with Envision Medical Solutions for services necessary to provide our programs and services to members; Basic administrative data is shared with the Envision Medical Solutions and is used by Health Plan Partners, LLC or Envision for administrative and customer service purposes, including but not limited to providing duplicate Member ID cards, drug pricing assistance, pharmacy locations, and program use instructions. All business associates adhere to a strict privacy and confidentiality policy.
Our Policy With Respect To Former Customers:
We follow the same privacy policies and practices for former members as are described in this notice for current members.
We safeguard any information provided to our company. In addition, we limit the collection and use of nonpublic personal information to the minimum required to provide members with superior products or service. We restrict access to information to those employees who need to know that information to provide products or services. We maintain physical, electronic, and procedural safeguards that comply with regulatory standards to guard any nonpublic personal information. All personally identifiable data is stored securely, and access is limited only to authorized administrative staff.
